HOPWA Outcome Assessment Screen Corrected

Overview
A recent issue was reported where the HOPWA Outcome Assessment summary screen in HMIS was showing Acuity Index fields that didn’t belong, adding confusion for end users. This made it difficult for staff to correctly enter HOPWA outcomes. The form has been updated to display only the HOPWA outcomes.
