2026 BOS Renewal Evaluation
BOS Participant Survey Deadline Extended
Due 2/2/26
Dear Providers,
Just a reminder that we have extended the deadline for BOS Participant Surveys and they are now due 2/2/26.
Participants may complete BOS surveys two ways:
- On paper: CT BOS 2026 Participant Survey – English or CT BOS 2026 Participant Survey – Spanish
- Electronically via Survey Monkey. Please ensure that participants do not fill out both. See 2026 Consumer Survey List for the link to your project’s survey.
Please note, if your agency participates in the DMHAS consumer survey process, we will be getting participant surveys directly from DMHAS. This year, we will be getting surveys from 1/1/25-6/30/25 from DMHAS. Therefore, any DMHAS surveys entered into their system after 6/30/25 will not be used as part of the BOS 2026 renewal evaluation process.
Please do not submit DMHAS surveys to BOS. If you receive DMHAS funding and intend to use BOS participant surveys instead of DMHAS surveys for CT BOS renewal evaluation purposes, please contact us to let us know that is your preference.
