CHANGES TO ACUITY INDEX COMMENTS FIELD

Change will appear on: 10/03/2022

Changes have been made to the comments field for acuity assessment outcomes. The comment field has been moved from the top summary section of the form to individual notes that are associated with each outcome. Important: These fields are not case notes.

• Case Management (Tab) > Left Menu: Program Assessments > Acuity Index Outcomes Assessment > Add New
