Streamlined Participant Chart Checklists
for CoC & other Housing Programs
Dear Providers:
The resources below have been streamlined and updated. Please take a moment to remove old versions of the forms and begin using these updated versions. Significant changes are summarized below:
- CoC Required Documents Checklist for PSH Housing Coordinators
- Combines items from HUD, CT BOS and DMHAS in one place & specifies optional/best practice items for non-DMHAS providers
- CoC Rapid Rehousing (RRH)File Checklist, Rapid Rehousing
- Combines items from HUD, CT BOS and DOH in one place and offers separate sheets for entry, annual recertification and exit
As a reminder, the resources below were were revised and released earlier this year. Please check that you are using the updated versions.
- Participant Rights Notice (all programs)
- Combines the Grievance and Emergency Transfer Rights Notices and the Participant/Applicant Bill of Rights into a single form
- Projects can now use just this single form rather than 3 separate documents.
- Consolidated No Income Certification (PSH, RRH (except Rapid Exit), and TH (except YHDP TH)
- Consolidates the PSH No Income Certification and the Zero Income Affidavit used for DOH Programs including RRH (except Div. Rapid Exit), Emergency Housing Vouchers (EHVs), Rental Assistance Program Vouchers (RAPs), Fairshare and Mainstream Vouchers
- Now projects will use the same form regardless of funding source.
- Health and Safety Checklist (PSH) – revised to simplify and make this process easier to complete
- Unified Authorization to Sign Form (PSH)
- Combines Owner’s Authorization to Sign, Partnership Agreement, and Corpprate Resolution into a single form.
- PSH projects can now use this single form instead of 3 separate documents.
- Documentation of income eligibility (TH) – The Steeting Committee voted to discontinue this requirement. TH projects no longer need to verify that the applicants are below 30% AMI at project entry.
Background Information
During the most recent CT BOS Strategic Plan update, the CT BOS team was asked to identify opportunities to reduce documentation burdens by streamlining and making requirements consistent across funders. In response, our team reviewed all documentation requirements across funders, met with the DOH and DMHAS teams and convened a provider work group to discuss opportunities to streamline. Most documents were determined to be HUD requirements, where feasible we streamlined. We hope these changes are helpful.
