Community Integration Sessions

The Connecticut Long Term Care Ombudsman Program (LTCOP) would like to invite supportive housing provider staff who may be either accepting or working with residents who are working on going to or coming from a Residential Care Home to participate in the upcoming Regional RCH Community Integration Roundtables being convened across the state this June.

These roundtables are intended to create space for Residential Care Home providers, Access Agency staff, waiver case managers, MFP assessors, and community partners to come together for practical, conversation-based discussions focused on strengthening coordination, communication, and shared understanding across systems.

 

The goal of these sessions is to support informed choice and improve collaboration related to Residential Care Homes as a community-based option, including helping individuals successfully transition into RCH settings and supporting them in remaining there as they choose to age in place.

These are not formal compliance trainings. Rather, they are intended to strengthen working relationships, improve understanding of available services and referral pathways, and provide an opportunity for providers and community partners to discuss real-world challenges and solutions together.

 

Topics will include:

• Understanding Medicaid waiver services and referral pathways available to individuals residing in Residential Care Homes

• Building relationships with Access Agency staff, MFP assessors, waiver case managers, and community partners

• Discussion of real-world cases and coordination challenges

• Strategies to support successful community transitions and aging in place

• Identifying barriers to placement, transition, and ongoing community supports

• Creating stronger regional communication and collaboration across systems
