Overview

The By Name List version 2.10 has been released including several changes that may impact use of the BNL.

Changes to the BNL Launch Form

The Form Field “CAN Filter (Enrollment)” has been renamed to “Coordinated Access Network*”, which reflects the column name affected by this filter.

Changes to “CAN Filter (Enrollment)” Field

  1. Help Text has been added for the field, “Coordinated Access Network*” to inform the user how this field has been calculated, this is indicated by the addition of the help icon () next to the field. The below text is available as of this release and outlines the

Addition of “Display Heads of Household Only” option

You can now remove all results that are not marked on the column “QP Relation To HoH“ as equal to “Self” from the BNL. This is unchecked by default.

Changes to the Coordinated Access (Input) Assessment

The “Coordinated Access (Input)” assessment has been updated with three new questions.

This assessment is completed as part of intake workflow for Coordinated Access Network (CAN) enrollments and is also accessible via the Case Management Tab > Left Menu: CAN Assessments > Intake Form (CAN).

The following fields have been added to the form and are displayed at Entry, During, Annual and Exit events.

  1. Does this client want to be housed in a different CAN? (Not Required)
     – Yes / No Options
  2. Which CAN does the client want to be housed? (Required if field 1 is set to Yes)
     –  Displays list of CANs when “Yes is Chosen” from above question.
  3. Subcommunity  (Shown if “Fairfield County Coordinated Access Network”, “Meriden-Middlesex County-Wallingford Coordinated Access Network”, or “Northwest Coordinated Access Network”)
     – Displays a list of subcommunities for some of the CANs

These fields can be used to override the Qualifying Enrollment CAN when filtering the “Coordinated Access Network* for clients being served while in one CAN but being housed in another CAN.

Changes to the Client Status on By Name List (BNL) Form

The “Changes to the Client Status on By Name List (BNL)” form has been updated to mark the fields “Transfer CAN” and “Subcommunity” Read-Only so no new changes can be made to the data in these fields. These fields have been replaced with those documented in the “Changes to the Coordinated Access (Input) Assessment” section of this document.

This form is accessible via the Case Management Tab > Left Menu: CAN Assessments > Client Status on By Name List (BNL).

Changes to the By Name List Report

The above form changes have impacted the By Name List logic and presentation in the following ways.

  1. The column “Coordinated Access Network*” has an updated formula logic.

Formula Logic

IF “Does this client want to be housed in a different CAN?” = “Yes” on the latest assessment contains data, display that data.

Database Location: XCoordinatedAssessment.X_DesiredToBeHousedDifCAN

IF “Does this client want to be housed in a different CAN?” is blank default the CAN to the value contained in column “Qualifying Enrollment CAN”

  1. The new column “Subcommunity” which is located next to “Coordinated Access Network*” has data provided by the “Subcommunity” assessment question.
    • Database Location: XCoordinatedAssessment.X_SubcommunityDesiredToBeHoused
  2. The fields “Transfer Can” and “CAN Sub Community” have been moved to the “Additional Troubleshooting Diagnosis Info” section of the report, showing as Yellow columns.

 

Coordinated Access Network Date*

        If Present:

        The new “Which CAN does the client wants to be housed (XCoordinatedAssessment.CANDesiredToBeHoused)”. LATEST data.

        If “Which CAN does the client wants to be housed (XCoordinatedAssessment.CANDesiredToBeHoused)” is not present, default to LATEST

        “Qualifying Enrollment CAN”

        A date column will be added to the report “Which CAN does the client wants to be housed (Assessment Date)”, to give context to how old that data is. Place it next to the “Coordinated Access Network*) column

 

Impact & Necessary Actions

Due to the effects of these changes, it’s recommended CANS review this release and update internal processes accordingly.
