Overview

We are excited to introduce the new CT HMIS Outcome Report, designed to provide comprehensive insights into client outcomes. This report is now available to CT HMIS (CaseWorthy) users with access to the roles listed to the right, offering a detailed view of outcomes in your program and associated client information.

Key Features:

Outcome Report Summary View:

  • Presents each outcome with a count of clients per outcome and a corresponding score, offering a quick overview of overall outcomes achieved within the specified time frame.

Outcome Report Detailed View:

  • Provides an in-depth list of clients and their associated details, including:
  • Client ID
  • Last Name
  • First Name
  • Birth Date
  • Relation to HoH (Head of Household)
  • Program Name
  • Enrollment Begin Date
  • Enrollment End Date
  • Enrollment ID
  • Outcome Domain
  • Outcome
  • Score Report Parameters Tab:
    • Displays the parameters entered by the user, ensuring transparency and easy reference for the specified report settings.

How to Access the CT HMIS Outcome Report:

  1. Log in to your CT HMIS (CaseWorthy) account.
  2. Choose your Role
  3. Click on Administration
  4. Click on reports and select ‘Operational Reports’
  5. Look for the new “CT HMIS Outcome Report v1” under the “Goal / Outcome Reports” section
  6. Select the desired time range and parameters, Organization, and Program/s.
  7. Generate the report to view comprehensive outcome details for each client.
