Overview
To comply with the updated DMHAS Standard File Format and reduce data upload errors into the DDaP system, updates were made to the DMHAS / DDAP assessment form.
An alphanumeric character validation and a maximum length limit of 9 characters have been applied to the following fields:

Insurance Policy Number 2

Insurance Policy Number 3

Insurance Policy Number 4
These fields now restrict data entry to only alphanumeric characters (letters and numbers) and will not allow more than 9 characters to be entered. This directly addresses formatting errors—such as the inclusion of dashes or spaces—that previously caused DDaP file uploads to fail.
When completing the DMHAS / DDAP assessment, users must enter insurance policy numbers (such as Medicaid IDs) using only letters and numbers, up to a maximum of 9 characters. The system will automatically block inputs longer than 9 characters. Please ensure staff stop using dashes or special characters in these fields.
