H2H Destination Report

Overview
The H2H Destinations Report was designed to track client progress from enrollment through housing and eventual discharge from H2H program
The report includes two components:
- Summary Report – Provides a high-level view of program performance, including:

- Average number of days clients are: Enrolled, Enrolled to Housed, and Housed to Discharged

- Enrollments that occur during or after their H2H enrollment

- Exit Destinations
It also breaks down clients into key groups:

- Leavers – Clients who exited the H2H program, including their HUD exit destinations
- Stayers – Clients still enrolled in H2H, including other program enrollments during participation
- Client Detail Report – Provides client-level detail to support validation and deeper analysis, including:

- H2H enrollment dates and enrollment ID
- Housing date
- Related program enrollments (during or after H2H participation)
- Exit destination information
Impact & Necessary Actions
- This report is intended for program monitoring and outcome tracking.
- Users should use:
- The Summary Report for high-level trends and performance
- The Client Detail Report for validation and deeper review
Key Notes for Interpretation:
- Columns labeled with “(Days)” are averages, not counts of clients
No action is required to begin using this report.
