The H2H Destinations Report was designed to track client progress from enrollment through housing and eventual discharge from H2H program

The report includes two components:

1. Summary Report – Provides a high-level view of program performance, including:

• • Average number of days clients are: Enrolled, Enrolled to Housed, and Housed to Discharged

• • Enrollments that occur during or after their H2H enrollment

• • Exit Destinations

It also breaks down clients into key groups:

• • Leavers – Clients who exited the H2H program, including their HUD exit destinations
  • Stayers – Clients still enrolled in H2H, including other program enrollments during participation

1. Client Detail Report – Provides client-level detail to support validation and deeper analysis, including:

• • H2H enrollment dates and enrollment ID
  • Housing date
  • Related program enrollments (during or after H2H participation)
  • Exit destination information
