Streamlined DMHAS/DDaP Street Outreach Assessment Workflow

Overview

The DMHAS/DDaP workflow within CT HMIS has been updated for all applicable Street Outreach and PATH programs. In collaboration with DMHAS EQMI, extensive data fields that are not strictly required for the homeless outreach level of care have been removed to reduce data entry burdens while ensuring DDaP upload compatibility.

The updated DMHAS / DDAP (Input) – SHORT v2 and H2H and SNOFO DMHAS/DDAP (Summary) V2 assessments have been fully deployed to production template workflows.

The following sections and data fields have been entirely removed or marked as not required:

  • Demographics & Screenings: Religion , Tobacco Use , and Co-Occurring Assessment Date.

  • Mental Health & Substance Abuse Screenings: Mental Health Screening Tools/Scores , Trauma/Suicidality/Gambling sub-questions , and Substance Abuse Screening Tools/Scores.

  • Socioeconomic Status: Employment Status , Highest Grade Completed , Dependents/Minors on Income , and Principal Income Source.

  • History & Living Situation: Living situation at time of episodic assessment , History of homelessness in the last 6 months , and Days in a controlled environment.

  • 30-Day Metrics: Number of arrests , Self-Help meetings attended , and Family/Friend social support metrics.

  • Clinical Assessments: The entire Mental Health Diagnosis Assessment (ICD-10 codes) , DSM Summary , Target Symptoms , and the extensive Drug Information/Method of Use matrix history grids.

Impact & Necessary Actions
  • Workflow Impact: Frontline staff will experience a drastically shortened entry workflow when enrolling clients into DMHAS Street Outreach programs.

  • Paper Intakes: Staff should immediately stop using old intake packets and transition to the newly updated “Short” version of the paper client intake form that matches this system revision.

  • System Action: No manual system re-configuration is required by individual agencies. Program setup scripts have automatically applied these assessment templates across all active production programs.
