Instructions
If a person refuses to sign the CT HMIS Release Of Information form, the person’s data can not be entered into the system.
If a client’s data is already in HMIS and their ROI has expired and they refuse to sign a new release their existing data does NOT come out of HMIS. The result will be that there can be no further updates to their existing data in HMIS
