Overview

DOH has confirmed with HMIS TA that PH to PH transfers are allowable using these guidelines listed below.

1. If client was housed in the original PH:

  • Their actual move-in date should be entered for the original enrollment and the original enrollment should be closed using the transfer date as the enrollment end date
    • The exit destination should reflect the client’s actual situation which in an RRH to RRH case will be “Rental by client with RRH or equivalent subsidy”
    • The new PH program start date will be the actual date the client was enrolled in the new project and prior residence should reflect the client’s actual situation which in the RRH to RRH case will be “Rental by client with RRH or equivalent subsidy”
    • The housing move-in date for the new program will equal the enrollment start date of the new PH program and the enrollment end date will be the actual date the client is exited from the program
    • Exit destination should reflect the client’s actual situation

2. If the client was not housed in the original PH program and is still living in shelter or an unsheltered situation:

  • The enrollment end date for the original enrollment will equal the date of the transfer and the exit destination will reflect the client’s actual situation from the HMIS exit destination list
  • There will be no move-in date to enter for the original program enrollment
  • The new PH program should select the prior residence that reflects the client’s actual situation
