Overview

The purpose of this document is to help users understand how data is pulled into the H2H report.

H2H Program Measures

Clients in Reporting Period

# of Clients (Total) LOGIC: Unique count of client enrollments within the reporting period.
# of Clients Enrolled in Reporting Period LOGIC: Unique count of client enrollments with an Enrollment Begin Date within the report period.
# of Clients Exited in Reporting Period LOGIC: Unique count of client enrollments with an Enrollment End Date within the report period.

1 At least 90% of H2H consumers had an immediate prior residence of unsheltered living situation 90% LOGIC: Count of Clients with a HUD Universal Prior Residence of:

1 – Emergency shelter, including hotel or motel paid for with emergency shelter voucher, Host Home shelter

3 At least 80% of consumers will not return to homelessness after or within 2 years. 80% LOGIC: Count of Distinct Client Records within Reporting Period who: 1. Exited from an H2H program, AND 2. Had an Exit Destination at program exit, or a Current Living Situation recorded after exit, that shows the client returned to homelessness.

Homelessness includes the following situations:

1 – Emergency shelter, including hotel or motel paid for with emergency shelter voucher, Host Home shelter

16 – Place not meant for habitation (e.g., a vehicle, an abandoned building, bus/train/subway station/airport or anywhere outside)

18 – Safe Haven

4 Successfully Permanently Housed directly from Homelessness 75% LOGIC: Unique Count of Exits with Perm housing destination where Exit is within reporting period OR the LATEST Current Living Situation Assessment where Client is still enrolled and has a PSH value for “Current Living Situation” within report period or:

10 – Rental by client, no ongoing housing subsidy

11 – Owned by client, no ongoing housing subsidy

21 – Owned by client, with ongoing housing subsidy

22 – Staying or living with family, permanent tenure

23 – Staying or living with friends, permanent tenure

26 – Moved from one HOPWA funded project to HOPWA PH

6D Income Decreased (Income has decreased as compared to entry within report period, excludes clients with NO income at Entry) LOGIC: Count of unique clients who exited between the report start and end dates and had a decrease in Total Monthly Income at exit (or latest assessment) compared to their income at entry. 14.29%

Current Living Situation Assessments

LOGIC:

Count of unique adult clients who exited between the report start and end dates and had a HUD Universal Data Assessment completed at entry that includes a Prior Residence.

Adults at Most Recent (Stayers)

Count of unique adult clients with their most recent service (that includes a Current Living Arrangement) dated between the report start and end dates, where the Current Living Situation is one of the selected values.

Adults at Exit (Leavers)

Count of unique adult clients who exited between the report start and end dates and had a HUD Universal Data Assessment at exit that includes a Prior Residence.
