Landscape Analysis Survey

Dear Colleagues:

We hope you are doing well and staying warm. The Technical Assistance Collaborative (TAC) has asked us to send out a reminder about their survey.

The Technical Assistance Collaborative (TAC) has been engaged, through HUD-funded technical assistance, to support Connecticut and both of its CoCs in improving outcomes for people with serious behavioral health and other complex conditions experiencing homelessness. This initiative aims to strengthen integration between health and behavioral health systems and the homeless system, and to improve coordination with key partners such as FQHCs, Community Mental Health Centers, and alcohol and drug treatment providers.

Thank you for taking the time to complete this Landscape Analysis Survey The questions are designed to gain your perspectives on the cross-system alignment needed to better meet the whole-person care needs of people experiencing homelessness with complex behavioral health and medical conditions. We anticipate that this survey will take approximately 25-30 minutes to complete and your response will help TAC and Connecticut’s CoCs strategically partner with these systems to end homelessness and promote health, well-being, and recovery for the individuals we serve.
