DMHAS Homeless to Housing RFP

Dear Providers,
DMHAS has issued a Request for Proposals (RFP) for a Homeless to Housing Program, new awards are expected to take effect July 1, 2027.
The purpose of this RFP is to develop programs to support people experiencing homelessness find and sustain stable, safe, permanent housing. Agencies will do this by implementing interdisciplinary service teams that address the individualized needs of each consumer. The service teams will follow the consumer from their unsheltered or sheltered situation through the stabilization of permanent housing. The program will follow a “services follows the person” model.
The Homeless to Housing RFP defines service requirements, performance measures, application and implementation timelines, and other related information.
