PIT Count 1-Day Reminder

+ Severe Cold Weather Protocol

 

Dear Colleagues:

As we prepare for the annual Point in Time (PIT) count tomorrow night, we are aware that there is also extreme weather impacting where people are staying, and additional resources in place due to the Severe Weather Protocol activation – in place from 1/23 through noon 1/28. This email provides information about the protocol in place as well as PIT resources and action steps.

Severe Cold Weather Protocol

Due to the extremely cold weather, anyone in need of shelter or a warming center is urged to visit 211ct.org or call 211 to find available locations. Transportation can be provided if necessary. While the Severe Cold Weather Protocol is enacted, a system is set up for agencies and municipalities to coordinate with United Way 211 and CT’s network of shelters to ensure that anyone in need can receive shelter from the outdoors, including transportation to shelters.

Provider Action Steps – Complete Before and on PIT Night

Outreach & Warming Center Programs

  • Exit anyone not seen in last 90 days
  • Complete Current Living Situation assessments for each contact

Emergency, Seasonal, Overflow Shelter & Hotel/Motel Programs

  • Shelters – ensure bed night check-ins are completed
  • Hotel/Motel programs:
  • Exit anone not contact in last 60 days
  • On PIT night, complete Hotel/Motel service in HMIS for head of household

Transitional Housing & Safe Haven

  • Confirm all active HMIS enrollments are accurate
  • Exit clients no longer in the program

PSH, RRH and PH Programs

  • Ensure move-in dates are completed for head of household in HMIS
  • Exit clients no longer in the program
