The Homeless Verification in CT HMIS Frequently Asked Questions has been updated and is now available on the CT HMIS website.
The FAQ provides guidance from DOH and DMHAS on the Homeless Verification functionality in CT HMIS, including how information is populated, who is responsible for updates, signatures, manual entries, and what to do when information does not appear as expected.
Important: Beginning October 1, 2026, all housing programs are required to use the Homeless Verification in CT HMIS instead of the paper form, with limited exceptions for certain YHDP programs and programs serving veterans or individuals who are not recorded in HMIS.
The updated FAQ also clarifies responsibilities after a participant is enrolled in housing. Prior to referral, outreach workers and shelter staff should provide updates as needed; after enrollment in RRH or PSH, the receiving housing program is responsible for necessary updates.
The FAQ is now available on the following CT HMIS program pages:
- Coordinated Access
- Permanent Supportive Housing
- Street Outreach
- Cold Weather Shelters / Warming Centers
- DMHAS SNOFO
- Emergency Shelter (ES)
- Homeless Prevention (HP)
- Homeless to Housing (H2H)
- Rapid Rehousing (RRH)
- Services Only
- Transitional Living Program (TLP)
